With a trifecta in hand, they have the opportunity to fulfill their promise of “massive” changes. However, the lack of clarity on their plans has left many wondering what’s to come.

A Key Factor: The Expiring Subsidies

One crucial aspect that could influence the Republican’s approach is the impending expiration of the ACA’s insurance subsidies, expanded by the Biden administration. These subsidies, set to lapse after 2025, have led to a significant increase in enrollment on the exchanges, with many lower-income families benefiting from zero-premium plans and limited out-of-pocket expenses. If allowed to expire, premiums and deductibles will surge, affecting millions of Americans.

The Impact on Coverage

According to the Kaiser Family Foundation, households currently pay 44% less for health plans than they would under the original law’s subsidy scheme. If the subsidies are not renewed, a single 45-year-old making $40,000 a year would pay an extra 67% for a mid-tier silver plan, or $1,247 a year. Larger, middle-class families could face thousands of dollars in additional costs. The Urban Institute estimates that the cost of coverage would increase by $2,900 for households earning more than 400% of the poverty line.

Consequences of Inaction

Experts predict that price hikes would lead to some Americans dropping their coverage, while others would downgrade to cheaper plans. The Congressional Budget Office estimates that 3 million more Americans would likely go uninsured. Sara Collins, a senior scholar at the Commonwealth Fund, warns that letting the enhanced marketplace premium tax credits expire would be a significant blow to low- and middle-income people who depend on them.

Potential Changes Beyond Subsidies

Republicans may go beyond simply letting the subsidies expire. House Budget Committee Chair Jodey Arrington has called for finding savings in federal health care programs, which could involve cuts to the ACA or Medicaid. Some lawmakers may seek to loosen regulations requiring insurance companies to cover certain benefits or allow companies to separate healthy customers and people with pre-existing conditions into separate risk pools. This could make insurance cheaper for many families but more expensive for those facing health problems.
